This course will take delegates through some principles of RCA and incident investigation processes, highlight pitfalls and challenges likely to be encountered in implementing effective RCA processes, and then provide a methodology for undertaking effective RCAs. This methodology will include how to get to true root causes without getting stuck at the superficial “human error” level.
